Abstract

: Toxoplasma infection or toxoplasmosis is an infectious disease caused by the parasite Toxoplasma gondii. Toxoplasma gondii infection of the central nervous system occurs in patients with immunodeficiency. The recommended first-line therapy is pyrimethamine + sulfadiazine + leucovorin, however, due to the unavailability of pyrimethamine, the alternative therapy that can be used is trimethoprim + sulfamethoxazole. We present a case of cerebral toxoplasmosis in a male patient, 31 years old, with HIV disease, admitted to the hospital with the main complaint, namely gradual disorder of consciousness (DOC) or decreased consciousness six days before entering the hospital. Initially, the patient looked confused. However, over time, the patient becomes increasingly challenging to communicate with. The patient is post-status general motor onset tonic seizure. The patient has had diarrhea for two months with black stools. The patient also had a fever. The patient received a combination therapy of Clindamycin 4x600mg and Cotrimoxazole 2x960mg. The combination of Clindamycin and Cotrimoxazole is recommended as an alternative therapy, but further research is needed regarding the outcome of therapy.
